I've got the Halo Triton, which uses the Kanga T2 tanks, if I'm not mistaken, that have the atomizer on top. The Halo branded atomizers have longer wicks then what Kanga originally supply's, so they do good at picking up off the bottom of the tank. So I normally don't pay to much attention to the direction.

I have heard the exhale through the nose tip a lot. I personally can't stand it. Doing that makes my nose feel wet and runny. To answer the OP though, I don't hold it in at all. Just breath it in and out like a normal breath. This means it starts coming out immediately, but slowly.
