Step 1 - Create a new layer
In Photoshop, open the image you chose to work with. To add the smoke, create a new layer on top. Do this by clicking on the small button at the bottom of the Layers panel. Rename the new layer to Smoke.
Step 2 - Create a Smoke Brush
We will start by creating the brush. Select the Brush tool and use a large and soft brush. We chose a brush about 300 Px with 0% Hardness and a 50% Opacity.
Step 3 - Create a path for the smoke
We need to make paths that mimic the smoke tendrils to apply the brush to. Select the Pen tool (P) and make sure that Path is the chosen format. With it, draw a wavy path where you want some of the smoke to be.
Step 4 - Combine the smoke brush with the path
To apply the smoke brush to the path, make sure that white is set as the Foreground color and that the Smoke layer is selected. Then, use the Path Selection tool (A) to right-click on top of the path and choose Stroke Path.
Step 5 - Add more smoke tendrils
Normally there would be several smoke tendrils. So, we need to create other paths and stroke them with the brush. To deselect the paths completely so you don’t see them, hit Esc.
Step 6 - Adjust the shape of the smoke
We need to make the smoke look less flat. Make sure that the Smoke Layer thumbnail is selected. Then click Command/Control + T to enter Free Transform.
Step 7 - Convert to Smart Object and apply Surface Blur
To add some non-destructive effects to your smoke we will turn it into a Smart Object. Right-click on top of the smoke layer thumbnail and choose Convert to Smart Object.
Step 1
Open all photos in Photoshop. Copy Smoke 1 photo to portrait canvas and position smoke photo like I did below.
Step 2
Desaturate smoke image by selecting the smoke layer and going to Image > Adjustments > Desaturate (Shift+Ctrl+U).
Step 5
Copy Smoke 2 to our main canvas. Repeat same actions with this smoke layer like you did with first one: desaturate it and set layer opacity to 70%. Finally, rotate this layer a bit by using Edit > Transform > Rotate.
Step 6
Apply Edit > Transform > Warp to replicate face shape. Regarding face, try to shape it as similar as possible.
Step 7
Select Smudge Tool with the soft-edged brush ( Strength: 50%) to improve contour. In the end, we should have something like this:

In order to depict the photo exactly, we should use several images of smoke as a rule. In this tutorial, I used only three images of smoke.
Step 9
Go to third smoke image and select part of the smoke using Polygonal Lasso Tool. Copy selected area to the main canvas and place it as shown on the screenshot below. Desaturate this layer by going to Image > Adjustments > Desaturate (Shift+Ctrl+U) and set layer opacity for this layer to 70%.

Before we jump in to the tutorial, here is a quick preview of what we will be creating:
Step 1
We are going to begin by creating a cloud brush. Download this free cloud photo and open it in Photoshop. Then make a rough selection around the area shown and copy it to the clipboard. You don't need to be too precise here, we will tidy it up later.
Step 2
Create a new document and set both the width and height to 2500 pixels. This is the maximum size for brushes in older versions of Photoshop and should be more than enough for our brush. Also set the background color to black.
Step 4
Select the Eraser tool and using a soft round brush clean up your selection. It's okay to leave some blue sky areas, but be sure to get rid of any hard white edges.
Step 5
Duplicate (CMD+J or CTRL+J) the cloud layer and from the Edit > Transform menu choose Flip Horizontal and then Flip Vertical. Move the duplicated layer down and with the Transform controls (CMD+T or CTRL+T) try to align the edges of the duplicated layer with the original to form an almost circle.
Step 6
Select the Healing Brush tool, choose a round brush preset and set the Hardness to 85% . Click and drag over the seams where the two sections of cloud meet. Repeat as necessary to blend together the clouds.
Step 7
Open the Channels window and click on the Red channel, then duplicate the channel.
